Understanding Your Insurance Benefits for 2026

As we begin a new year, we want to help our patients understand an important change that happens with most insurance plans.

Insurance benefits reset on January 1, which can affect deductibles, copays, and coinsurance—even if your care and insurance plan remain the same.

This update applies to patients with Medicare, Medicare Advantage, and Commercial insurance plans.


What Does a “Benefit Reset” Mean?

At the start of each calendar year:

  • Annual deductibles restart

  • Copays or coinsurance amounts may change

  • Out-of-pocket maximums reset

As a result, patients may notice higher out-of-pocket costs early in the year.


Medicare (Original Medicare – Part B)

For patients with Original Medicare:

  • Medicare Part B has an annual deductible that resets each year

  • For 2026, the Medicare Part B deductible is $283

  • After the deductible is met, Medicare typically pays 80% of the approved amount

  • Patients are responsible for the remaining 20% coinsurance

If you have secondary insurance, that plan may cover some or all of the 20% coinsurance depending on coverage. Secondary insurance payment is not guaranteed, and patients remain responsible for any unpaid balance.


Medicare Advantage (Part C) Plans

Medicare Advantage plans are offered by private insurance companies and may include:

  • Per-visit copays

  • Coinsurance percentages

  • Annual deductibles

  • Prior authorization requirements

Coverage and costs vary by plan, so it’s important to review your specific benefits.


Commercial Insurance Plans

Commercial insurance plans may include:

  • Annual deductibles

  • Copays per visit

  • Coinsurance percentages

Even if you paid very little at the end of last year, benefits and deductibles may reset on January 1, 2026.
